PMS & PMDD: When Premenstrual Symptoms Affect Your Life
Premenstrual syndrome (PMS) refers to physical and emotional symptoms that occur during the days or weeks before your period and typically improve shortly after menstruation begins.
Premenstrual dysphoric disorder (PMDD) is a more severe form of premenstrual disorder. Mood symptoms can be significant enough to interfere with work, relationships, school or everyday activities.
Common Symptoms of PMS & PMDD
You may experience:
- Irritability or mood changes
- Anxiety or feeling overwhelmed
- Depression or low mood
- Increased emotional sensitivity or crying
- Fatigue or reduced energy
- Difficulty concentrating or brain fog
- Sleep changes
- Food cravings or increased appetite
- Bloating or fluid retention
- Breast tenderness
- Headaches or migraines
- Acne or skin changes
- Abdominal cramping
- Joint or muscle discomfort
With PMDD, emotional symptoms such as severe irritability, depression, anxiety or mood changes tend to be more pronounced and can significantly affect daily functioning.
Not every symptom that occurs around menstruation is necessarily caused by PMS or PMDD. Thyroid conditions, anemia, nutrient deficiencies, medication effects, sleep problems and mood disorders can produce similar symptoms and may require further investigation.
How Are PMS & PMDD Diagnosed?
PMS and PMDD are primarily identified by the timing and pattern of symptoms across the menstrual cycle. Keeping a daily symptom record for at least two menstrual cycles can help determine whether symptoms consistently occur during the premenstrual phase and improve after menstruation begins.
There is no single blood test that diagnoses PMS or PMDD. Testing may still be recommended when appropriate to investigate other possible contributors, such as:
- Thyroid dysfunction
- Iron deficiency or anemia
- Vitamin and mineral deficiencies
- Blood-sugar abnormalities
- Other health concerns suggested by your symptoms
Our Approach to PMS & PMDD Treatment
Treatment is individualized according to your symptoms, menstrual cycle, medical history, diet and overall health. Depending on your needs, your plan may include:
- Dietary changes
- Strategies to improve blood-sugar stability
- Assessment for nutritional deficiencies
- Targeted vitamins and minerals when appropriate
- Assessment of possible food sensitivities or dietary triggers
- Digestive and gut-health support
- Herbal medicine selected according to your symptoms
- Exercise and lifestyle strategies
- Support for sleep and stress
- Laboratory testing when appropriate
- Coordination with your family doctor or other healthcare providers when needed
Our goal is to identify modifiable factors that may be contributing to your symptoms and develop a practical treatment plan rather than using the same supplement or dietary program for everyone.
For patients with severe PMDD, naturopathic treatment can be used alongside appropriate conventional medical or mental-health care. Severe depression, suicidal thoughts or concerns about personal safety require prompt medical assessment.
