Perimenopause: When Hormonal Changes Begin

Perimenopause is the transition leading up to menopause. It commonly begins during a woman’s 40s, although symptoms can start earlier. During this time, estrogen and progesterone levels may fluctuate considerably, and menstrual periods often become less predictable.

Common Signs of Perimenopause

You may experience:

  • Changes in the timing or heaviness of your periods
  • Hot flashes or night sweats
  • Difficulty falling asleep or staying asleep
  • Fatigue or reduced energy
  • Anxiety, irritability or mood changes
  • Brain fog or difficulty concentrating
  • Headaches or migraines
  • Weight gain or changes in body composition
  • Breast tenderness
  • Reduced libido
  • Vaginal dryness or discomfort during intercourse
  • Joint or muscle discomfort

Not every symptom experienced during midlife is necessarily caused by perimenopause. Thyroid conditions, iron deficiency, sleep disorders, medication effects and other health concerns can produce similar symptoms. Heavy, prolonged or unusual bleeding may also require further investigation.

How Is Perimenopause Diagnosed?

Perimenopause is usually identified from your age, symptoms and changes in your menstrual cycle. A single hormone test is not always helpful because estrogen, progesterone and FSH can fluctuate substantially from one day or month to another.

Testing may still be recommended when needed to investigate other possible contributors, such as thyroid dysfunction, anemia, nutrient deficiencies, blood-sugar changes or cardiovascular risk factors.

Our Approach to Perimenopause Treatment

Treatment is individualized according to your symptoms, medical history, menstrual pattern, health risks and preferences. Depending on your needs, your plan may include:

  • Nutrition and lifestyle strategies
  • Support for sleep, mood and stress
  • Resistance training and cardiovascular exercise
  • Supplements or botanical medicines when appropriate
  • Assessment of bone, cardiovascular and metabolic health
  • Treatment for vaginal and urinary symptoms
  • A discussion of the potential benefits and risks of menopausal hormone therapy

For women experiencing troublesome hot flashes, night sweats or sleep disruption, hormone therapy may be an appropriate option after an individualized assessment. When prescription treatment is indicated, our clinic can provide or coordinate hormone therapy within the Ontario naturopathic scope of practice and collaborate with your family doctor or nurse practitioner when additional medication is required.

Menopausal hormone therapy does not prevent pregnancy. Contraceptive needs should therefore continue to be considered during perimenopause.
